- • Compress at 100-120/min, 1/3 chest width depth. Allow full recoil.
- • Ventilate 10 breaths/min once intubated.
- • Swap chest compressor every 2 minutes - rescuer fatigue hits hard.
- • Epinephrine 0.01 mg/kg IV/IO every other cycle (~ every 3-5 min). High dose 0.1 mg/kg only after multiple failed low doses.
- • Atropine 0.04 mg/kg for asystole or vagally mediated bradycardia.
- • Pause compressions for < 5 sec to check rhythm.
Frequently asked questions
Why 2-minute compression cycles?
Rescuer fatigue degrades compression quality measurably after 90-120 seconds. The 2-minute cycle is the longest interval where you can maintain effective rate and depth. Beyond that, swap compressors even if the patient hasn't regained ROSC.
How often should epinephrine be given?
Per RECOVER: low-dose epinephrine (0.01 mg/kg IV/IO) every 3-5 minutes - effectively every other compression cycle. High-dose epinephrine (0.1 mg/kg) only after multiple failed low doses, as it can worsen post-arrest outcome.
Should I stop compressions to check rhythm?
Yes, briefly - under 5 seconds at the end of each compression cycle. Restart immediately if no organised rhythm. Don't pause to set up ETT placement or vascular access; do those alongside ongoing compressions.
